The person in Itachi's stomach is Naruto.
This world needs more Itachi pred material.
Why does Sasuke look so shocked? Well, imagine coming home to seeing your brother's stomach bulged like that and your best friend's necklace (that you have given him) on the floor not too far from him. I think you'd have that expression too. The wolf and fox ears are major fails and don't ask why Naruto has still gotten a pair of human ears too. I guess I was too lazy to erase them ^^'
